USC Annual Physical Examination (APE) Schedules
February 13 & 14, 2011 Downtown Campus, VPAA Conference Room – LGB

February 21 & 22, 2011 South Campus, Campus Clinic
February 23 & 28, 2011 North Campus, PAASCU Headquarters
Insular Health Care Members will have the following laboratory exams: Complete Blood Count (CBC), Chest X-ray, Urinalysis, Stool Exam, Physical Examination (PE), Papsmear (Female ) and ECG for 35 years old and above. For this year’s APE, Insular Health Care has contracted the services of Hi-Precision Diagnostics.
Note: Fasting Blood Sugar (FBS) screening is not included in the HMO package. Any member who would like to undergo this test and other lab tests not included in the package during the on-site APEare advised to see their doctor for the issuance of referral and secure LOA from IHCare Coordinators before the APE day.
Non-Insular Health Care Members who have served the University for at least one (1) year counted from the start of probationary employment will be accommodated during the on-site APE free of charge for the basic 5 tests only: CBC, Xray, PE, Urinalysis and Stool Exam. Hi-Precision Diagnostics will be provided with a list of these employees. Those not in the list will still be accommodated but the cost of the APE will be charged to their personal account. For clearance purposes, the basic five (5) tests are required. Hi-Precision is charging Php350.00 for these tests. Salary charge slips will be available at the venue.
Part-time employees may avail of the on-site APE charge to their personal account. However those who are employed with another company and who may have recently underwent an APE, will only need to provide their respective campus clinic a copy of their recent annual physical examination results for clearance purposes.
Newly-hired employeesas of 2nd semesmter 2011-2012 who have complied with the pre-employment medical examinations are no longer required to go through the APE.
Additional laboratory tests may also be availed of upon request at separate costs. We have attached a list of laboratory tests and their corresponding cost, including the fasting guide for blood tests. Please inform the medical team upon registration.
Employees who will miss the above schedule will be accommodated at Hi-Precision Diagnostics, #28 WES Bldg. J, Llorente St., Cebu City, telephone 2560151-53.from March 1 to March 31, 2012.
